Your right to a Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your care will cost.

Under federal law, health care providers must give patients who don’t have insurance, or who are not using insurance, an estimate of the expected charges for medical services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services, including related costs such as evaluations and reports.
  • Make sure your provider gives you a Good Faith Estimate in writing at least one business day before your appointment. You can also ask any provider for one before you schedule.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Keep a copy or photo of your Good Faith Estimate.

Speech EZ is a private-pay practice, so this applies to every client. Your estimate is prepared individually once we know which services are recommended — the ranges on our pricing page are a guide, not a substitute for your own estimate.

Notice of Privacy Practices

This notice describes how health information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Protection of your health information. Your health information is kept private in accordance with the federal privacy regulations under the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information is information relating to your past, present, or future care, including evaluations, therapy notes, and treatment history.

How your information may be used. Your information may be used for treatment, for payment, and for health care operations — for example, coordinating your care, preparing invoices or superbills, and maintaining records. Information is disclosed to others only with your written authorization, or where the law requires it.

Your rights. You have the right to inspect and request a copy of your records, to request corrections, to request restrictions on how your information is used, to request confidential communication, to receive a list of certain disclosures, and to receive a paper copy of this notice.

Questions or complaints. Contact us at [email protected]. You may also file a complaint with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.

Paperwork you’ll receive

Before your first session, your therapist will send you an intake packet to complete and return. It includes consent to treatment, the financial policy, the attendance and cancellation policy, and optional release forms. These are signed directly with your therapist — nothing is submitted through this website.

Fees, session length, and any travel fee are confirmed with you before your first appointment.

Two independent practices

Speech EZ, LLC (Kelsey Timler, M.S., CCC-SLP) and Brandi at Speech EZ LLC (Brandi Porter, M.S., CCC-SLP) share this website and the Speech EZ name. They are separate businesses — separately owned, separately insured, and each responsible for its own clients and records. Neither has access to the other’s client information.

When you use the contact form on this website, your message goes directly to the practice covering your area and is not copied to the other. If you email our general address instead, it reaches Speech EZ, LLC, who will pass it on to the other practice if your area is theirs.

Once you begin services, that practice provides its own paperwork and is responsible for your records.
